Please note, your insurance web site might not have updated information as to whether they are contracted with Dr Tara Tarantino. Please call the office, if you have any questions abotu coverage.

We are excited to announce that all yearly forms will be sent via email or text to complete electronically before your appointment or sooner if you’d like to request the forms be sent any time.

New patients will receive forms to complete before scheduling your first appointment.

Please bring the following to every appointment:

  1. Insurance Card(s)

  2. Picture Identification (such as a driver’s license or passport)

  3. You might also like to bring a current medication list and any imaging or blood test results that may not be in your John Muir Health/BASS Medical Group chart
